Our mission is to provide first class quality physical therapy services, promote wellness, prevention, and progressive care in an outpatient setting. As well as provide a choice for physical therapy closer to home and school in northwest Madison County.

INSURANCE/NEW PATIENTS

We accept nearly all major insurance plans. 

We accept Blue Cross Blue Shield, Medicare, Medicaid, Aetna, Tricare, Humana, VA, and Worker's Compensation Insurance. If you are a new patient please fill out the form below and bring it with you to your initial evaluation.
